This session will be a multidisciplinary primer for medication safety in the ICU setting. Many practitioners hope to provide patient care in an atmosphere free from medication errors but are unaware of standard processes to proactively ensure medication safety. Speakers will address medication safety from the perspective of a critical care clinician who might have become aware of recent serious medication errors covered in the press and would like to learn best practices for preventing medication errors in the ICU. Motivation from recent major medication errors will be used to highlight the need for proactive medication process evaluation.

Learning Objectives:

Identify methods to promote a culture of medication safety in the ICU setting
Demonstrate the steps involved in performing a failure mode and effects analysis in the ICU to prevent medication errors
Compare and contrast different methods of preventing medication errors and improving patient safety with a multidisciplinary approach
