Title: Transcultural Adaptation for Universal Spanish Version of a Critical Care Functional Assessment Tool
Background: In today's globalized world of OECD era, there is an increasing need to have transversal measurements or tools that enable comparison of indicators and trend data. Therefore, the relevance of having unique versions of evaluation instruments, universally adapted in a particular language.
Objective: To perform the transcultural adaptation (TA) of an English functional assessment tool for Critical Care patients (CPAx) to a universal Spanish version using digital/remote resources.
Methods: With the original author´s permission, professionals from 21 Spanish speaking countries were invited to participate. An expert panel was convened locally, their coefficient of expertise was determined centrally. The original and first Spanish version of CPAx tool was send for expert`s judgment for consensus through an iterative process using Likert scale (LS) and the Delphi method guided by digital work forms based on international guidelines. In a second phase, usefulness, acceptability, safety and quality of the adapted version were assessed through pre and post online questionaries (Cronbach Alfa, Mc Donald´s Omega) and validity and reliability through pilot testing (rater- interrater Intraclass Correlation Coefficients).
Results: In 2018, 90 multidisciplinary health professionals (MHP) of 15 Spanish speaking countries (71.4%) participated in the first step, achieving face validity of the instrument after 3 Delphi rounds. In 2019-2020 due to the pandemic 44 MHP of 13 countries participated in the Pilot Test including 102 patients with 1,831 evaluations. Pre and post questionaries mean rating (of 1 to 6 LS rated questions) were 5.20±0.64 and 5.23±0,67 respectively (t test pns) Cronbach Alfa and Mc Donald´s Omega were both 0.925 and 0.90, respectively. 91% of participants rated the tool as very good or excellent on both occasions, 94% stated it offered advantages over similar tools and 35% referred a few limitations: needs a dynamometer, amount of time and personnel needed. The ICC rated from 0.79 (CI95% 0.64-0.93) to 0.99 (CI95% 0.97-1.00) and in 60% of the sites it was over 0.9.
Conclusions: This pre-pandemic initiative illustrates the usefulness of health informatics for collaborative teamwork beyond geographic boundaries to achieve the TA of a single, reliable, safe universal Spanish version of CPAx for Critical Care.
