Hannah Chan, PharmD is the clinical pharmacy specialist in the emergency department at Baptist Hospitals of Southeast Texas. She received her Doctor of Pharmacy degree at the University of Houston College of Pharmacy in Houston, Texas. After receiving her PharmD, Hannah went on to pursue a PGY1 Pharmacy Practice residency at Memorial Hermann – Texas Medical Center. After completion of her PGY1 program, Hannah stayed on at Memorial Hermann for her PGY2 residency and specialized in the field of Critical Care. Hannah has a collaborative practice agreement for antimicrobial stewardship with the Emergency Department Medical Director at Baptist Hospital of Southeast Texas. Hannah is also an active member of the Society of Critical Care Medicine. Additionally, Hannah serves as a member of the Pharmacy & Therapeutics, Antimicrobial Stewardship, Sepsis, and Trauma/Emergency Services committees.
