My name is Colleen Badke and I am a pediatric intensivist and Instructor of Pediatrics at Ann & Robert H. Lurie Children’s Hospital of Chicago/Northwestern University Feinberg School of Medicine who is committed to using a data-driven approach with continuous physiological data to develop real-time prediction models that can identify clinical deterioration, risk-stratify critically ill children, and support medical decision making. In the pediatric intensive care unit where I work, and in many similar PICUs across the country and the world, intensivists take care of children of all ages with a broad range of medical problems and variable acuity. When caring for these critically ill children, we often use heuristics and subjective interpretations, along with a limited set of easily accessible objective data (such as vital signs, physical examination, and ancillary studies) to determine the severity of illness of a particular child. We use these tools to attempt to predict the trajectory, clinical course, and prognosis for each child -- will they improve rapidly, worsen but recover over time, or progress to death? I believe that we can harness the massive amounts of data collected by bedside monitors to improve this clinical decision making. I am passionate about the early detection of clinical deterioration because I believe that if we detect a worsening trajectory with enough advance notice, we can improve prevent avoidable morbidity and mortality in critically ill children.
